Align's capsule contains one specific strain, Bifidobacterium 35624, and that strain has real trial data behind it in irritable bowel syndrome (IBS), though the picture is mixed. The best-known trial found a benefit over placebo for abdominal pain and bloating at one dose[1], but later pooled analyses are mixed[2][3], and the main US guidelines do not endorse probiotics for IBS[4][5].

This review covers what is in each Align product, what the 35624 trials actually measured, what guidelines say, side effects, and who might reasonably try it. It looks at the evidence for a strain, not at how a particular box feels to take.

How we reviewed this

We read the manufacturer's product, dosing and safety pages in October 2026 for the label facts below[6][7][8]. We read the abstracts of the published 35624 trials and of the guidelines and meta-analyses that assess them. We did not test the product hands-on, and this page carries no affiliate links.

Be aware of who funded the research. Two trials listed Procter & Gamble affiliations among their authors[9][10], and three open-label studies involved authors who were paid by, employed by or sponsored by Biocodex or the strain's maker[11][12][13].

What is in Align, and which products contain 35624?

The 24/7 Digestive Support capsule contains Bifidobacterium 35624 and nothing else active. Other Align products add ingredients or swap the probiotic.

Product Probiotic per the manufacturer Other ingredients
24/7 Digestive Support (capsule) 35624[6] None listed as active
Bloating Relief + Food Digestion (capsule) 35624[6] Vitamin B12 and digestive enzymes[6]
Women's Dual Action (capsule) 35624 plus two other probiotics[6] Chaste tree botanical[6]
DualBiotic Prebiotic + Probiotic Gummies Bacillus coagulans[14] Prebiotics

That last row matters: the gummies do not contain 35624, so the trials below do not apply to them. The same goes for any product that adds other strains. For a wider look at women's formulas, see our best probiotics for women guide.

The manufacturer states each capsule has at least 1 x 109 CFU (4 mg) when made and at least 1 x 107 CFU through the best-by date[7]. It needs no refrigeration, and the label lists milk and sodium caseinate[6][8]. The strain was formerly named Bifidobacterium infantis and is now classed as Bifidobacterium longum[15]. You can read about the species on our Bifidobacterium longum page.

Does Align work? What the trials found

Align's strain has beaten placebo in controlled trials of people with IBS, but not in every population or at every dose. The controlled evidence comes from two older randomized trials, and the newer studies are open-label.

Study Design Main finding Caveat
Whorwell 2006 362 women with IBS in primary care, 4 weeks, placebo and three doses 1 x 108 cfu beat placebo for abdominal pain, bloating and gas; global relief exceeded placebo by over 20%[1] 1 x 106 and 1 x 1010 doses did not differ from placebo[1]
O'Mahony 2005 77 people with IBS, 8 weeks, 1 x 1010 cells in a malted milk drink Greater reduction in pain, bloating and bowel difficulty than placebo[16] Small; a drink, not a capsule[16]
Sabaté 2022 233 evaluable patients, 30 days, 1 x 109 daily, open-label Mean severity score fell from 303 to 208; 57% moved to a lower severity category or remission[11] No placebo; authors reported personal fees from Biocodex[11]
Lenoir 2023 37 adults, 8 weeks, open-label Total symptom score fell 43.4%[12] Tiny, no placebo; authors included Biocodex employees[12]
Cruchet 2024 64 children aged 8 to 18, 12 weeks, single-arm Symptom scores improved; gains were not kept during a two-week washout[13] No placebo; sponsored by a company that later became the strain's maker[13]

Two points stand out. The pivotal capsule trial found its effect at 1 x 108 cfu, not at the higher dose, and its authors warned that products should be tested in their final form and dose[1]. And open-label studies cannot separate the probiotic from the placebo response, which is large in IBS.

Is Align worth taking for bloating if you do not have IBS?

Probably not for severity. A multicenter trial randomized 302 adults with bloating and discomfort who had not sought care for it, giving 1 x 109 cfu daily for 4 weeks[9]. Both groups improved, and end-of-trial severity scores did not differ between probiotic and placebo[9]. Only the number of bloating-free days was higher on the probiotic[9]. The authors suggested a high placebo effect and lower symptom burden as explanations[9].

If gas and bloating are your main concern, our guide to probiotics for bloating covers other causes and options.

What do guidelines say about probiotics for IBS?

Guidelines are cautious. None endorses a specific brand.

  • American Gastroenterological Association (2020): use probiotics for IBS only in the context of a clinical trial[4].
  • American College of Gastroenterology (2021): suggests against probiotics for global IBS symptoms (conditional recommendation, very low-quality evidence)[5][17].
  • British Society of Gastroenterology (2021): it is reasonable to let patients who want to try one do so for up to 12 weeks and stop if there is no improvement (weak recommendation, very low-quality evidence)[18].

The pooled data explain the caution. A 2023 meta-analysis of 82 trials (10,332 patients) rated certainty as low to very low across almost all comparisons, with low certainty of benefit for abdominal pain with Bifidobacterium strains[3]. A 2017 meta-analysis of five trials found a single-strain 35624 product did not change abdominal pain, bloating or bowel satisfaction, while composite products containing it did (pain effect size 0.22, bloating 0.30)[2]. A 2026 strain-level review of single-strain placebo-controlled trials, however, found efficacy for 35624 on key IBS symptoms[15]. In short, the evidence leans positive but is not settled.

For subtype-specific options beyond this strain, see our best probiotics for IBS guide and the IBS overview.

How does strain 35624 act in your gut?

It passes through and leaves. In an 8-week trial, fecal levels of the strain rose with daily capsules and fell back toward baseline once dosing stopped, so colonization was transient[10]. Only limited changes appeared in 10 other bacterial groups, and that trial saw no effect on IBS symptoms, so changes in the stool microbiota are not a good measure of whether it works[10].

An earlier trial proposed an immune mechanism: people with IBS had an abnormal ratio of the cytokines IL-10 and IL-12, and the ratio normalized with the strain[16]. Treat this as a hypothesis, not proof of how any capsule works for you.

What are the side effects of Align, and who should skip it?

Side effects are usually mild. The manufacturer reports no significant difference from placebo in clinical studies, with some people noticing gas and bloating in the first days[8]. A meta-analysis of 55 trials found adverse events were not significantly more common with probiotics than placebo[3].

Ask a healthcare professional first if any of these apply[8][19]:

  • you are severely ill or immunocompromised (harms from probiotics are more likely in people with severe illness or weakened immunity);
  • you are pregnant, breastfeeding or trying to conceive;
  • you have a milk allergy or lactose intolerance, since capsules contain milk and sodium caseinate;
  • you take an antibiotic, because some may make Align less effective, or you take any prescription medicine.

Align is not meant to replace digestive medicines[7]. Premature infants face a separate risk: the US Food and Drug Administration has warned of severe, sometimes fatal infections from probiotics in them[19].

How do you take Align, and how long does it take to work?

The manufacturer's advice is one capsule once a day at any time of day, and no doubling up after a missed day[7]. Keep capsules in the original bottle at room temperature[7].

For duration, the trials give a guide: 4 weeks in the pivotal trial[1], 8 weeks in two studies[16][12], and 30 days in the large open-label study[11]. The BSG suggests trying a probiotic for up to 12 weeks, and stopping it if you see no improvement[18]. Track your symptoms for a week beforehand so you can judge.

Align vs. Culturelle: which has better evidence?

Both have some evidence in IBS, in different strains. Align's capsule uses 35624. We did not retrieve Culturelle's label this run, so check which strain your Culturelle product lists. The 2026 strain-level review found efficacy in pooled placebo-controlled trials for both 35624 and L. rhamnosus GG on key IBS symptoms[15]. We did not find a head-to-head trial of the two in the sources we read, so neither can be called better. Learn more on our Lactobacillus rhamnosus page.

Who might consider Align, and who should look elsewhere?

It may be reasonable to try if you have a clinician-diagnosed IBS with pain and bloating, you want a single, defined strain, and you will track symptoms for 4 to 12 weeks.

Look elsewhere or talk to a clinician first if you have not been assessed for IBS (for suspected IBS with diarrhea, the ACG guideline suggests celiac serology and fecal calprotectin to rule out celiac and inflammatory bowel disease)[5], you are immunocompromised, or you want a product for a purpose the trials did not test. For broader comparisons, see our best probiotics overall page.

This article is for general educational purposes and is not medical advice. Product details were retrieved from the manufacturer's website in October 2026 and may change; always read the current label. Talk to a qualified healthcare professional before starting a probiotic, particularly if you are pregnant, immunocompromised, taking prescription medication, or managing a diagnosed condition.

Frequently Asked Questions

Is Align a good probiotic for bloating?

Possibly, if your bloating is part of IBS. In a 4-week trial of women with IBS, strain 35624 at 1 x 10^8 cfu beat placebo for bloating and abdominal pain. In a later trial of 302 adults with bloating who had not been diagnosed with IBS, severity scores did not differ from placebo, although bloating-free days were more frequent on the probiotic.

Is it bad to take Align every day?

The manufacturer designs Align for once-daily use. Trials of strain 35624 ran 4 to 12 weeks, and the British Society of Gastroenterology suggests trying a probiotic for up to 12 weeks and stopping it if symptoms have not improved. No long-term safety trials of daily use were among the sources we read, so check in with a clinician if you plan to take it for months.

Who should not take Align probiotics?

Ask a clinician first if you are severely ill, immunocompromised, pregnant, breastfeeding or trying to conceive. The label lists milk and milk protein (sodium caseinate), so people with a milk allergy should check with a clinician. The US National Center for Complementary and Integrative Health notes that harms from probiotics are more likely in people with severe illness or weakened immune systems.

What medications should you not take with probiotics?

Align's manufacturer says some antibiotics may make Align less effective and advises talking to your doctor if you take one. It also says Align does not replace digestive medicines and that you should check with a healthcare provider if you take prescription medication. Never stop a prescribed medicine to start a probiotic.

Is there a better probiotic than Align?

It depends on the symptom. Other single strains, such as Lactobacillus rhamnosus GG and Lactiplantibacillus plantarum 299v, also had positive pooled results in a 2026 strain-level review of IBS trials. We did not find a head-to-head trial against Align, so no brand can be called better. Guidelines rate the overall evidence for probiotics in IBS as low to very low certainty.

How much does Align probiotic cost at Walmart?

Retail prices at Walmart and other stores vary by capsule count and formula and change often, and we did not retrieve current store prices, so we do not quote one. Compare the price per capsule on the shelf, and remember that the 24/7 Digestive Support capsule and the gummies contain different probiotics.

Is Align probiotics worth the money?

That depends on whether your situation matches the research. The strongest controlled data for strain 35624 are in people with IBS, where guidelines rate the overall evidence for probiotics as low to very low certainty. In adults with bloating who did not have an IBS diagnosis, one trial found no difference in symptom severity from placebo. A time-limited trial of up to 12 weeks with symptom tracking is a reasonable way to judge value.

Who are the three people who should not take probiotics?

There is no official list of three. Sources we read single out three groups for extra caution: people who are severely ill or immunocompromised, premature infants (the FDA has warned of severe infections), and anyone allergic to an ingredient in the product, such as the milk protein in Align capsules. Ask a clinician before starting if any apply.

Is there a probiotic that contains Bifidobacterium?

Yes. Bifidobacterium and Lactobacillus are the most common bacteria used in probiotics, according to the US National Center for Complementary and Integrative Health. Align's 24/7 Digestive Support capsule contains Bifidobacterium strain 35624, while its gummies contain Bacillus coagulans instead.
